Intraoperative Demarcation of Tumor Borders by 5-ALA-induced Protoporphyrin IX Fluorescence for the Resection of Malignant Glioma

Abstract

Malignant gliomas selectively accumulate Protoporphyrin IX (PPIX) 5 hours after oral delivery of 20 mg/kg 5-aminolevulinic acid (5-ALA). Intraoperative fluorescence imaging discloses vital tumour tissue by its red fluorescence with a high contrast to normal brain. A Zeiss varioscope was equipped with a sensitive 3-chip CCD color camera to enable multicolor fluorescence imaging as well as standard white light imaging. Fluorescence guided resection led to augmental removal of tumor tissue as compared to white light surgery. Biopsies from normal brain were completely negative in PPIX-fluorescence (44 biopsies).
